When you schedule your first appointment, we'll send you the New Patient Intake form along with other forms for you to complete. Completing these forms prior to your appointment will maximize your allotted treatment time. The intake form will seem like a lot of questions, but they are all important to understand your overall health status in order to develop an effective treatment protocol.
